1. Information we collect
Information you give us: your name, email, phone, date of birth, shipping address, state of residence, payment details (processed by our payment provider), and answers to health questionnaires.
Information from your devices: if you choose to connect Apple Health or Health Connect to our tracker app, the sleep, heart rate, temperature and activity data you allow.
Information collected automatically on our marketing pages: device type, browser, pages viewed and similar usage data, using cookies and similar tools, as described in section 4.
2. How we use information
To provide your account, membership, care coordination, prescriptions and shipping through our clinician and pharmacy partners.
To process payments, send receipts and renewal reminders, and respond to support requests.
To improve our website and app, keep them secure and comply with the law.
We do not sell your personal information, and we never use your health information for advertising.
3. How we share information
With [Medical group, P.C.] and its clinicians, so they can evaluate and treat you.
With [Pharmacy partner] and lab partners, to fill prescriptions and process tests you order.
With service providers who help us run Her Venus (hosting, payments, email and text messaging, customer support), under contracts that limit their use of your information and, where HIPAA applies, business associate agreements.

4. Ads and analytics
We may use cookies and advertising tools only on our marketing pages (such as our homepage, care pages and blog), and only with your consent where required.
We do not place advertising pixels, conversion tools or ad tags on our health questionnaire, checkout, account, treatment detail pages or tracker app. Your health questionnaire answers, diagnoses, prescriptions and tracker data are never shared with advertising platforms.

5. Your rights and choices
Depending on where you live, you may have the right to access, correct, delete or get a copy of your personal information, and to opt out of certain uses. You can make a request in your account or by emailing [privacy@ email].

6. How we protect your information
